Care provided:

Our team provides 24/7 two-to-one support to ensure Tania’s safety, stability, and consistent care within the community. The focus of care is on restoring Tania’s independence, dignity, well-being, and confidence in everyday life.

Team members supporting Tania are trained in Type 1 Diabetes management, including blood glucose monitoring, insulin support, and responding appropriately to health concerns.

The team administers prescribed medication and supports Tania with daily routines, meal planning, and glycaemic-safe eating.

We support Tania to attend health appointments and work closely with her family and clinical teams to ensure care remains coordinated and responsive to her needs.

We assist Tania in building daily living skills, including household tasks, shopping, and safe community engagement.

During periods of distress, our team use calm, trauma-informed de-escalation strategies to maintain safety, preserve trust, and reduce escalation.

Through consistent staffing, strong relationships, and a stable support environment, we have helped rebuild Tania’s trust and stability following previous placement breakdowns.
