Community-based transitional housing, Wellington region

Care provided:

Our team has provided high-needs support to Andrew in a safe, community-based transitional housing setting for nearly four years.

We initially provided two-to-one support for Andrew, which has since reduced to one-to-one support as his stability, confidence, and engagement have improved.

The team supported Andrew through the transition into a new placement following previous service breakdowns, providing the consistency and reassurance needed to help him settle.

Staff developed clear and consistent daily routines to reduce anxiety and minimise behavioural escalation.

They worked closely with Andrew’s whānau to maintain trust, continuity, and a shared understanding of his needs.

We also provide safe transportation to appointments, educational activities, and community access, helping Andrew remain connected and engaged.

When incidents occur, staff respond immediately using calm, trauma-informed de-escalation approaches that prioritise safety, dignity, and emotional regulation.

Daily incident reporting is provided to multidisciplinary providers and funders to ensure transparency, coordination, and responsive care planning.

We support positive behaviour strategies that focus on empowerment.

These strategies enabled Andrew to move into his own apartment, obtain workplace qualifications, and get his Learner Driver’s License.

Through consistent staffing, strong rapport, and relationship-based support, Andrew has shown improved emotional regulation, trust, and engagement in daily life.
